Admission Criteria
- The guest is in the last weeks to months of life, as determined by the resident’s physician, hospice team, and review by LaVigne Home staff.
- The LaVigne Home is not a skilled facility and cannot accept guests that require care skilled care. The care needs must be able to be met in a non-medical setting and must not exceed LaVigne Home staff’s capability. Staff cannot provide care for a guest requiring IV medication, IM medication, a feeding tube, tracheostomy, breathing machines, blood sugar monitoring, insulin injections, or bariatric services.
- Urinary catheters and oxygen are acceptable.
- The guest must be enrolled in a local hospice program of their choice and stay admitted throughout the stay. If an individual does not have a hospice company, a list will be provided to the patient and/or family.
- The guest’s and/or caregiver’s behavior must not present a danger to self or others and would not be disruptive to others at LaVigne Home.
- The guest must not have known, active TB. Other active infections will not be excluded, however evaluated individually.
- Meals are provided; however, some diet choices may not be able to be implemented. Food of the patient’s choice may be brought in by family or friends for the patient.
- The guest or health care power of attorney must have an established “Do Not Resuscitate” Status.
- The guest and/or family member understands the purpose of admission and the plan of care, including understanding of a discharge plan if needed.
- The guest and family understand that there is no charge for care at the LaVigne Home. When there is a question of bed availability, a decision of admission will be based on those of greatest need. Staff of the LaVigne Home reserves the right to deny admission to guests with active, infectious diseases that may place other guests or staff at risk.
